How Many Amalgam Fillings Can Be Removed in One Visit?

It is one of the questions I am asked most often, usually within the first five minutes of a consultation. "I've got eight of these silver fillings. Do I have to keep coming back, or can you take them all out in one go?"
The honest answer is that it depends. But it depends on far fewer things than you might imagine, and almost none of them are the things people worry about.
First, why the pace of removal matters at all
Dental amalgam is roughly half mercury by weight. That is not a controversial claim, it is simply what the material is. When an amalgam filling is drilled, mercury vapour and fine particles are released, and research consistently shows that removal without protective measures produces a temporary spike in exposure for the patient and, for that matter, the dental team.
This is precisely why the SMART protocol exists. SMART stands for Safe Mercury Amalgam Removal Technique, a set of engineering controls developed by the International Academy of Oral Medicine and Toxicology, the IAOMT. Rubber dam isolation, high-volume suction placed at the tooth, a separate clean air supply for you to breathe, external air filtration, sectioning the filling out in chunks rather than grinding it to dust, and copious water throughout. Each element is designed to do one job, to minimise how much of that vapour and particulate you take in while the filling comes out.
Think of it like removing old asbestos lagging from a pipe. The material sitting there quietly is one thing. Disturbing it carelessly is another thing entirely. So you don't disturb it carelessly. You seal the area, you control the air, and you take it out properly.
So, one visit or several?
Here is the part that surprises people. Once the protective measures are properly in place, the question of one visit versus several becomes largely a practical one rather than a safety one, because the protections apply every single time the drill touches amalgam. The rubber dam does not care whether it is your first filling of the day or your sixth.
In my own practice on Harley Street, both approaches happen every week, and the deciding factors are usually these:
How many fillings you have, and where they are. Amalgams are removed under rubber dam isolation, typically working through the mouth in sections. Two or three fillings in one quadrant is a comfortable single appointment. Ten fillings spread across all four quadrants is a longer day, but with adequate appointment time it is entirely achievable in one sitting.
How far you are travelling. A significant number of my patients come to London from across the UK, from York, from Scotland, from the South West. For them, one carefully planned extended appointment is far more sensible than four separate train journeys. We plan these cases meticulously in advance so that nothing is rushed on the day.
Your comfort. Some people are happy to settle in for a longer appointment and have it all done. Others prefer shorter visits, particularly if dental anxiety is part of the picture. Both are legitimate. My job is to make either option calm and unhurried, not to push you towards the one that suits my diary.
The teeth themselves. Occasionally a tooth under an old amalgam has other plans, deep decay, a crack, the need for a crown rather than a simple replacement filling. That is assessed before we start, not discovered halfway through, which is exactly why every case begins with a full assessment.
What about the "one quadrant at a time" rule?
You may have read online that amalgams must only ever be removed one quadrant at a time, with weeks between visits. It is worth being straight about this. There is no high-quality clinical evidence establishing a mandatory pace of removal. The staged approach is a reasonable and common way to work, and I offer it happily. But when the full SMART protections are in place at every appointment, I am not aware of good evidence that a properly planned single visit is less safe than the same work spread across four. What matters is not the calendar. What matters is what is protecting you while the drill is running.
What removal will and won't do
I hold myself to a simple standard here, and it is the same one I use in my forthcoming book. I will tell you what the evidence shows, and I will not dress it up.
What I will not tell you is that removing your fillings will cure any particular illness. The research on health outcomes after removal is still emerging, and anyone who promises you a transformation is overstepping what the science currently supports. My commitment is narrower and, I think, more trustworthy: if these fillings are coming out, they should come out in the safest way the profession currently knows how. That is what SMART is for, and it is what I have spent years teaching other UK dentists to do.

It depends on the number and position of the fillings, which is assessed in advance. Extended appointments are booked with generous time so nothing is hurried.
